---tllhe Appalachia Community Cancer Network (ACCN) proposes to develop and pilot a cancer patient
navigation (PN) program for its community cancer coalitions, partners, and rural hospitals. The goal of the
ACCN PN program is to reduce the burden of cancer in Appalachia by providing support to patients with an
abnormal finding on a breast, cervical, and colorectal screening and to ensure that patients receive timely
diagnostic, treatment, and survivorship care. The ACCN PN program will provide a service that is not present
in many medically underserved Appalachian communities, which contributes cancer health disparities for
people in Appalachia. 1
