North Fork Valley Community Health Board and the University of Kentncky (UK) College of Medicine as co-operators of the North Fork Valley Community Health Center (NVFCHC) are seeking a Section 330 satellite new access point grant to establish a health care center (June Buchanan Medical Clinic) in Knott County that will offer quality health care to those in need in Southeastern Kentncky. The co-applicant arrangement will continue for the operation and oversight of this New Access Point. The proposed health center will operate from the conversion of a rural health clinic previously managed by Appalachian Regional Healthcare, which had been used for direct medical care in Knott County. The Knott County Satellite New Access Point will offer new services to our adjoining county, in accordance with our vision of becoming a vital and responsive community fixture in the health care landscape of Eastern Kentncky. This new satellite access point will provide quality health care through an integrated system of health care providers. Services will include comprehensive women's health care encompassing obstetrical care and family planning, pediatric acute and well child care, newborn care, geriatric care, and immunizations for both children and adults. The health center will also offer a range of services like physicals for preemployment, sports physicals, and workman's compensation exams. Supplemental services will include case management, health education, low-cost pharmacy and transportation assistance. The community health center will also provide mobile oral health services through use of an mobile dental van (for the pediatric dental population) and a fixed dental site (to serve the adult population). Mental health services will also be available on-site.
