Abstract The UK Prevention Research Center (PRC) will conduct program evaluation activities for the Kentucky Breast and Cervical Cancer Screening Program. Specifically, the UK PRC will lead the evaluation and performance measurement plan, including data plan; lead the health systems implementing evidence-based interventions (EBIs) strategy; and collaborate to identify areas in program implementation that need to be changed or improved by examining and monitoring the Program’s outcome data. The UK PRC will also conduct the following activities: 1. Conduct qualitative and quantitative data analyses based on grant needs, and report at monthly meeting. 2. Monitor and evaluate activities that support strategies outlined in the work plan 3. Ensure health disparities are addressed in the development of the evaluation plan/ data plan 4. Monitor and evaluate Performance Measure data outlined in the work plan 5. Submit timely, the required evaluation plan(s) to funder (CDC) 6. Collect and submit health systems’ clinic-level data per CDC requirements 7. Maintain databases and perform quantitative and qualitative analyses on baseline and annual clinic data, and EBI assessments 8.Conduct, at least quarterly, health system conference calls evaluating EBI progress 9. Collect and track outreach data, and assist data manager on strategies outlined in workplan 10. Develop visuals, survey instruments, and newsletter sections based on the evaluation results
