Capacity building for Health Outcomes Inspired by Community Empowerment (CHOICE)
Goal 1: Explore perceptions of the COVID-19 vaccine among individuals living in low-income,
diverse communities in Fayette County by hosting two community-engaged listening sessions
each year.
Goal 2: Recruit and engage at least 10 adult and 10 youth CHOICE Community Champions to
participate in a 12-month health equity leadership development training program to support
tailored community-based health communication and health equity initiatives.
Goal 3A: Implement school-based and after-school programming to reach a minimum of 250
Fayette County youth living in low-income, diverse communities each year.
Goal 3B: Implement two community tailored health fairs/outreach events each year to reach
individuals living in low-income, diverse communities.
Goal 3C: Develop and disseminate an innovative and community tailored health
communication/social media outreach campaign (disseminated monthly at minimum) to
provide accurate information about vaccines, including the COVID-19 vaccine, and access to the
vaccine among individuals living in low-income, diverse communities in Fayette County.
Goal 4: In collaboration with community partners, host one vaccine clinic quarterly (estimated
6 total) at local community centers which serve individuals living in low-income, diverse
communities in Fayette County.
