ABSTRACT Fiscal Year 2021 Building on the success of My Choice Kentucky to-date, we propose to expand the project to focus on groups and individuals who have been slow to embrace supported decision-making (SDM). School systems personnel and family member of school-age children will specifically be targeted. We will create materials specific to each group that will explain SDM as a viable alternative to guardianship and share personal success stories. Additionally, we will endeavor to present at least once at a relevant conference and offer trainings to individuals and groups. The project will continue to provide trainings and technical support to other groups and individuals, as requested. These requests continue to increase as word of the project and its successes spread. We expect to conduct six (6) trainings to various groups (e.g., guardianship, advocates, service providers).These presentations will be completed either as stand-alone events to groups or as part of larger conferences at the local, state, or national level. In concert with our project partners, Kentucky Protection & Advocacy, we will offer technical assistance to the court system as people seek rights restoration. If needed, we will also provide education to support teams of those seeking rights restoration. Additionally, we will be available to provide expert testimony regarding SDM during restoration hearings. We will continue to capture the stories of people in Kentucky who are using SDM. During this round of funding we could like to get stories for not only those who have used SDM in their personal lives as well as those who have seen SDM used in their professional lives. These professionals could include case managers, direct support professionals, officers of the court, and guardians. We would like capture stories of five individuals in this fiscal year.
