Description

The Foundation shall undertake the following obligations: • Receive water samples delivered by the Cabinet as agreed to in advance by the Kentucky Geological Survey so that QA/QC standards are not violated. A Chain-of- Custody (COC) form completed by the Cabinet that shall serve as the instructions for the analyses required shall accompany each sample. • Maintain the custody and integrity of the samples at all times and shall store the unused portion of each sample for a period of three (3) months after the sample collection date. • Perform the following tasks as defined by the COC for each sample. These tasks include: a. Preparation of water samples for all laboratory analyses. b. Analyze the prepared samples for the constituents specified on the cac document for each sample. • Perform additional analysis of water samples for the Cabinet according to item 1.3 of this AGREEMENT. • Using the lists "Analysis Parameters and Costs"(Attachment II), prepare and submit an invoice to the Cabinet for each sample. Nitrogen-isotope extractions shall be billed at the cost of seventy-five dollars ($75) per extraction. The Foundation shall submit invoices according to item 5, Method of Payment, for this AGREEMENT. • Report the analytical data to the Cabinet in a fonnat suitable for electronically loading into the Department for Environmental Protection Consolidated Groundwater Database, and in hard copy to include the completed analyses together with the documentation necessary to validate the results. Reports are to be submitted to the Cabinet within sixty (60) days of receipt of each sample.
