Description
The University of Kentucky Regional Extension Center (KY REC) shall provide support to the Chronic
Disease Prevention Branch (CBPB) in order to improve Diabetes Outcomes among defined target
populations by providing technical assistance to CDPB and designated partner health systems which
include completing gap assessments in the use of Electronic Medical Records for screening data, data
validation onsite audits, creating clinic baseline data worksheets, tracking practice plan-do-study-act
(PDSA) work to improve measures and conducting a Diabetes Learning Collaborative for Diabetes quality
improvement.
