Description
The Kentucky Department for Public Health (DPH) received a grant from the Centers for Disease Control
and Prevention (CDC) to work with health systems to implement evidence-based interventions, support
strategies to increase cancer screenings and conduct quality improvement learning collaboratives. This
work will include working with health systems, including Federally Qualified Health Systems and clinics
that provide services and primary care, including colon cancer screening opportunities. Utilizing
electronic health records to develop maximum opportunities to identify and navigate these persons to
colon cancer screening in order to prevent cancer will reduce the burden and mortality of colon cancer
in Kentucky.
