Community Outreach and Change for Diabetes Management (COACH 4-DM)

The Kentucky Public Health Research Network (K-PHReN) will accomplish the following activities for this research project over a two year period. Develop and implement a systems-based quality improvement (QI) program Provide training for five research study coordinators Provide 3 QI training sessesion to each participanting DCOE Provide continual guidance Coordinate 1-2 tele-video conferences Collect pre and post internvention data on the QI process Analyze data for presentation at regional and national public health meetings Be an active participant in the National Public Health PBRN Program
