Description

This agreement is to provide $23,838 in newly awarded SAMSHA federal grant funding for the Kentucky Medication Algorithm Project (KyMAP) to the University of Kentucky, School of Nursing. As the "Fidelity Assessment Coordinators" for KyMAP, the School of Nursing will be responsible for insuring that on-site fidelity assessments are conducted as detailed in the scope of services of this agreement. Payments terms: Monthly, upon receipt and approval of vendor invoice. Total amount of this agreement is not to exceed $23,838
