Description
Under contract, the Kentucky Farmworker Outreach Program agrees:
To provide bilingual information to inform low income Spanish speaking families about
KCHIP and other program benefits via the use of a statewide 800 telephone number and
written material, and educate them about appropriate use of health services.
Provide a monthly outreach activity report using format agreed upon with KCHIP to
include types of phone calls received, and resulting follow-up activities.
Meet biannually with designated KCHIP staff to discuss the activities and needs of the
program.
Distribute KCHIP enrollment information and other informational material in Spanish as
requested.
Assist in the translation of KCHIP material to Spanish upon written request of the
Department for Medicaid Services including the KCHIP website.
F. Use the KFOP program 800 line to receive and log in Spanish speaking calls forwarded
from existing hotline numbers for Louisville and Fayette County health departments.
G. Assist with application and re-certification process for Spanish speaking population.
