Description
Person-centered case planning puts the person at the heart of the planning process and
emphasizes their unique needs, preferences, and aspirations. The Department for Aging
and Independent Living identi?es the need for a person-centered approach in the services
it provides.
The training will be developed from the Person-Centered Toolkit which was developed by
the LifeCourse Nexus Training and Technical Assistance Center in partnership and with
support from the National Center on Advancing Person-Centered Practices and Systems
(NCAPPS). NCAPPS is funded and led by the Administration for Community Living and
administered by the Human Services Research Institute (HSRI).
This project will (1) develop and deploy and in-depth training course on how to deliver
person-centered services using the competency-based learning plan in the Person-
Centered Competencies Toolkit (page 12), (2) develop with key stakeholders a tool used by
supervisors to assess the competencies across the three levels of performance (page 18 in
the Person-Centered Competencies Toolkit), and (3) conduct interviews across the state to
capture positive person-centered experiences.
