Enhancing Patient Navigation at University of Kentucky Markey Cancer Center ABSTRACT: University of Kentucky Markey Cancer Center (UKMCC) is the only National Cancer Institute (NCI)-designated Cancer Center in Kentucky. UKMCC is accredited by the Commission on Cancer (CoC) with their highest rating, the Outstanding Achievement Award. A significant challenge experienced in UKMCC’s PsychOncology navigation program is a high volume of patients with psychosocial distress limited ability to address social determinants of health (SDOH) that pose barriers to timely and equitable access to care for cancer patients and their families, in particular socially disadvantaged patients. Another challenge is the lack of tools for tracking and reporting metrics for continuous quality improvement. The overall goal of the proposed project is to enhance UKMCC’s existing PsychOncology navigation program by augmenting innovative technology-based tools, increasing our capacity to improve health equity through addressing SDOH and utilizing continuous quality improvement. This goal aligns directly with the focus of the American Cancer Society (ACS) Request for Applications, which is to advance high-quality cancer care through capacity building and support for innovative, sustainable models of oncology patient navigation. We will integrate existing and enhanced components of UKMCC’s PsychOncology navigation program using what our team calls a Comprehensive Connected Cancer Care (C4) Navigation Model. The C4 Navigation Model is a multi-level approach we propose to use that would leverage digital technology and combine psychosocial screening, patient and family navigation, remote patient monitoring, and patient education for. The project objectives are to: 1. Carry out a co-design and iterative quality improvement process with stakeholders to integrate the C4 Navigation Model into UKMCC’s navigation processes and systems 2. Evaluate implementation of the C4 Navigation Model in UKMCC’s PsychOncology navigation program using robust process and outcome metrics 3. Plan for long-term sustainability of the C4 Navigation Model in UKMCC’s PsychOncology navigation program and future scalability 4. Engage in ACS’s multi-institutional learning community of grantees to access expertise and share best practices, results, and lessons learned
