Description
Patients with multiple chronic conditions (MCC) spend a great deal of time and energy on their care. Yet sometimes this work exceeds patient capacity - effort, time, emotion, help, and attention - when combined with other important tasks in their lives. When patient work exceeds patient capacity, non-adherence ensues. Rather than optimizing care for each chronic condition, we propose focusing on each patient's health situation and on the limited capacity patients can mobilize to enact proposed treatments and act to advance their health situation.
