Description

Research will be conducted to generate a single assessment tool with the greater effectiveness and outcomes for individuals with waivers. The specific aim of this contract is to assist the DMS with evaluation of eligible individuals of the programs and with assessment of an individual’s ability to care for themselves, develop systems for tracking evaluations and transitions into the Community Based Waiver program. Provide professional expertise, technical assistance, and guidance on providing comprehensive assessments and data analysis for the frail and elderly population and the physically disabled across the Commonwealth for the Home and Community Based Services (HCB) Waiver. Through the collection of the comprehensive assessment process, data will be collected and analyzed to determine needs and services for the waiver population. This data will be used to validate the HCB waiver tools and collection information for the outcome measure established within the Waiver for compliance with the Centers for Medicare and Medicaid Services.
