SECTION 2-SCOPE OF WORK
2.OServices Req11ired
The University of Kentucky (UK) shall:
I. Provide genetic services, through a m inimum of two (2) outreach sites in eastern Kentucky. These outreach sites shall provide a total of six (6) cl i nics per year, with a minimum of 40 clients served in the six (6) clinics.
2. Provide genetic services, through hospital consults and center-based clinics, with a minimum of 250 clients served.
3. Provide laboratory and diagnostic support services in the clinics. This refers to medical services that may be provided for patients seen in these clinics.
4. Bill third party payors, as appropriate, for patients covered by other sources of fund ing. For patients at I 00% or below the poverty level and patients above I 00% of the poverty level (based upon their particular need) offer discounts in accordance with the University of Kentucky's current Financial Assistance Pol icy.
5. Ensure all patients receive services under this contract regardless of ability to pay insurance co-pays, out-of-pocket expenses, or fees for services.
6. Collect genetic testing results for outreach and center-based services incl ud i ng: number of encounters (patient visits), undupl icated patients, location of service, date of service, sex, race, date of birth. residency and ICD-10 diagnostic codes. This information shall be submitted to the Division of Maternal and Child Health (timelines for reporting is provided in the Reporting Requirements section of this contract). (Effective July l, 2016 th rough June 30, 2017)
Collect genetic testing results for outreach and center-based services and report to the Division of Maternal and Child Health as outlined in the Reporting Requirements section of this contract. (Effective July 1, 2017 through June 30, 2018)
7. Schedule and provide at least four (4) education presentations to public health. med ical, and/or social work professionals. and/or students.
8. Provide medical consultation to the Division of Maternal and Child Health for Kentucky Birth Surveillance Registry (KBSR).
9. Provide referrals for children under the age of three (3) who are suspected to have developmental delays to the First Steps program. Kentucky's early intervention services.

5. Genetic services shall be provided to at least 250 clients at the University of Kentucky between July I.

6. Genetic services shall be provided for a total of 40 clients for the fiscal year in 2 outreach clinic sites in eastern Kentucky between July I , 2016 and June 30, 2017. (Effective July l, 2016 through June 30, 2017)
1.02-Reportilig Requirements
Effective July l, 2016 through June 30, 2017
UK shall submit the following to the Division of Maternal and Child Health: I . A schedule of outreach clinics on a quarterly basis.
2. Clin ic rosters for each month including identification of clients that did not keep their appointment shall be submitted by the 15th day of the following month.
3. Reports summarizing genetic testi ng results for center-based and outreach clinics including number of encounters. undupl icated patients. location of service, date of service. sex, race, date of birth. residency and ICD-10 diagnostic codes shall be submitted on a quarterly basis.
