Description

The University of Kentucky (UK) shall: 1. Provide genetic services, through two (2) outreach sites in eastern Kentucky. These outreach sites shall provide a total of five (5) clinics per year, with a minimum of 32 clients served in the five (5) clinics. 2. Provide genetic services, through hospital consults and center-based clinics, with a minimum of 200 clients served. 3. Provide laboratory and diagnostic support services in the clinics. This refers to medical services that may be provided for patients seen in these clinics. 4. Bill third party payors, as appropriate, for patients covered by other sources of funding. For patients at 100% or below the poverty level and patients above 100% of the poverty level (based upon their particular need) offer discounts in accordance with the University of Kentucky’s current Financial Assistance Policy. 5. Ensure all patients receive services under this contract regardless of ability to pay insurance copays, out-of-pocket expenses, or fees for services. 6. Collect genetic testing results for outreach and center-based services and report to the Division of Maternal and Child Health as outlined in the Reporting Requirements section of this contract. 7. Schedule and provide at least four (4) education presentations to public health, medical, and/or social work professionals, and/or students. 8. Provide medical consultation to the Division of Maternal and Child Health for Kentucky Birth Surveillance Registry (KBSR). 9. Provide referrals for children under the age of three (3) who are suspected to have developmental delays to the First Steps program, Kentucky’s early intervention services. 10. Ensure staff comply with all provisions of this contract. UK shall: 1. Increase access to genetic services for individuals and/or families in need regardless of ability to pay throughout the Commonwealth of Kentucky. 2. Increase the knowledge of primary care providers and other health care professionals about appropriate referrals and basic genetic knowledge. 3. Improve case ascertainment (identification of cases of an exposure or health outcome in public health surveillance) of children with birth defects, genetic and/or disabling conditions by reporting to KBSR and improve entry into early intervention services by referring children under the age of three (3) who are suspected to have developmental delays. 4. Ensure a minimum of five (5) outreach clinics shall be provided throughout central and eastern Kentucky between July 1 2018, and June 30 2019, providing services to a minimum of 32 clients. 5. Provid genetic services to at least 200 clients at the University of Kentucky between July 1, 2018, and June 30, 2019.
