Abstract - Kentucky Post School Outcomes Center Grant The stated project includes three components: Post School Outcomes; Parent Involvement; and SSIP Evaluation. The project as a whole will: 1). Oversee collection of post-school outcome data; analyze post-school outcome data; report results of analysis; provide technical assistance and training to local education agencies and the KDE regarding how to interpret the data and how they can be used for program improvement. 2). Utilize extant data from Kentucky Department of Education (KDE) and other sources to identify best practices related to transition and share these with stakeholders. Also explore opportunities for assessment of longer-term outcomes. 3). Chair collaborative efforts across agencies to identify and implement best practices related to transition for youth with disabilities. 4). Provide a facilitator that will provide ongoing technical assistance and follow up support across all of the Kentucky Department of Education / Office of Special Education and Early Learning (KDE/OSEEL) and University of Kentucky/Human Development Institute (UK/HDI) collaborative endeavors. The Facilitator will provide guidance for the project management team; serve as liaison between the staff at UK/HDI and the KDE; and facilitate planning meetings requested by OSEEL. 5). Oversee collection of parent involvement data; analyze data; report results of analysis. 6). Provide evaluation support to the State Systemic Improvement Plan for Indicator 17 which includes contributing to the Annual Performance Report, providing Implementation Team Reports and Dashboards, and monitoring progress on the State Identified Measurable Result (SiMR).
