FY21 UK Human Development Institute/KWCSP - Disability & Women's Health

The Kentucky Inclusive Health Collaborative (KIHC) at the University of Kentucky Human Development Initiative will partner with KWCSP for outreach to women with disabilities across the state who need cervical and/or breast cancer screenings, identify existing barriers to care, connect them to screening services, and conduct follow up outreach. In order to do this, the KIHC will implement a developed accessible assessment tool to identify womenfs health and screening needs within the female disability population through partnerships with local and state organizations that reach a broad audience of women with disabilities. Women with disabilities identified through the assessment will be referred to screening services available from KWCSP. For women connected to KWCSP services, follow-up post referral process will be conducted to ensure proof that cycle of care is completed or identify additional barriers to successful screening access. KIHC will work with KWCSP to collect MDEs on all KWCSP eligible participants, and patient navigation. KIHC staff will ensure follow-up post referral process are conducted and conformation that cycle of care is completed and/or identify additional barriers to successful screening access. This outreach will help gather data about disparities in cancer care for women with disabilities, identify potential barriers to obtaining necessary screenings, and assess the impact of geographic location on access to care. Once the data is collected and analyzed, it can then be leveraged to address any identified barriers to screenings for women with disabilities across the state with the continued support of the KIHC team. Additional efforts include information dissemination of outcomes from the assessment implementation, include training and education for KWCSP healthcare providers about serving people with disabilities, broadening outreach of mobile screening and education services to reach the areas of the state which experience higher cancer screening disparities, and creation of more accessible cancer education resources and programs for individuals with disabilities. In order to assess effectiveness of this funding effort we will gather the following metrics: ] Number of assessments completed ] Number of respondents who qualify for KWCSP services ] Number of geographical locations across state that identify higher need for services for women with disabilities ] Number of qualified individuals who need referrals ] Number of women who did receive follow up care following a referral ] Number of minimum data elements (MDE) records completed
