    Description

    Abstract The UKRF KY-REC will collaborate with the KOCHW in KDPH, partners on the CCR-2109 leadership team; community health workers and organizations employing CHWs to develop a learning collaborative focused on CHWs developing skills for Plan-Do-Study-Act (PDSA) and completing a story board. KY-REC will bring together the collaborative to discuss messaging, referrals for resources, and development of collaborative skill building. KY-REC will provide subject matter experts to assist CHWs and health care organizations in optimal use of the electronic health records in use in organizations within the collaborative for documentation of social determinants of health, COVID-19 vaccine messaging, and assist them to develop plans to maximize their use to identify appropriate patients for referrals and follow-up.
