University of Kentucky Research Foundation (UKRF) , KCP East shall: 1. Provide technical assistance/training to 2 health systems participating in the CDC funded “2002” grant to implement grant objectives related to the use of small media materials using resoures from the American Cancer Society, Centers for Disease Control or the National Colorectal Cancer Roundtable. Materials should be tailored to the needs ot the patient poplation served by the health systems. 2. Attend monthly virtual and/or in-person meetings of the Centers for Disease Control and Prevention (CDC) 20-02 grant leadership team to report on progress and challenges in delivering support to the designated health systems. 3. Participate in monthly health system TA support calls on a schedule to be mutually determined when the contract is in place. 4. Participate in monthly calls with Department for Public Health (DPH) staff to review progress toward program deliverables – call schedule to be determined when contract is in place. 5. Operationalize and implement the strategic outreach plan to promote Kentucky Colon Cancer Screening Program (KCCSP) to reach target population and health care providers serving that population. This plan was approved the Kentucky Colon Cancer Screening Advisory Committee in October 2019. 6. Collaborate with the Kentucky Cancer Link staff to coordinate joint outreach efforts to inform Kentucky residents about free colon cancer screenings through the Kentucky Colon Cancer Screening Program, elegibility standards for the program and how to contact KCL staff to enroll. Coordinate meetings with existing community coalitions to disseminate information on KCCSP. 7. Provide trainings to to community health coalitions and community stakeholders to educate partners about the KCCSP initiative and garner community support to reach under or un-insured Kentuckians.
