The Kentucky Cancer program will work with community health care providers and other organizations to provide tailored small media messaging to support provider/patient recruitment, breast cancer screening education and linkages to community support resources to facilitate access to screening and reduce barriers to care. The RCCSs will promote KWCSP services being offered in participating health systems (e.g. FQHCs) among the targeted population and will work with KWCSP in health system’s implementation plan development. The RCCS’s will initiate Community-Clinical Linkages strategies and collaborate with partners to ensure a focused coordination of services for the identified/designated KY providers.
