    ABSTRACT SOUTHEAST CENTER FOR AGRICULTURAL HEALTH & INJURY PREVENTION (SCAHIP): Third KY legislative appropriation (November 1, 2022 through June 30, 2023) SCOPE OF WORK Services Required SCAHIP will advance community referral capacity through the 1) the funding to contract with LivingWorks Applied Suicide Intervention Skills Training (ASIST) to provide 10 agricultural community members gatekeeper (train-the-trainer) and participant training. These 10 agricultural community members will provide in pairs a total of 5 ASIST trainings during Appropriation Year 3. Both gatekeeper and participant trainings will be researched and evaluated using mixed methods. A gatekeeper comfort measure and the CAMS developed program assessments will be used for these evaluations; 2) the funding of Collaborative Assessment and Management of Suicidality (provided by CAMS-Care) training to 25 agricultural community clinicians to advance their ability to work collaboratively with clients who have thoughts of suicide. Trainings will be evaluated to assess the effect of CAMS on nurse and counselor suicide assessment and management competency. Program coordination for both trainings will be provided by the BDHDID KY state agency. Terms & Conditions SCAHIP shall: 1. Submit invoices along with a short progress report monthly to the Commonwealth. All invoices must include the contract number, details of the total charge, receipts and/or payroll reports documenting the expenses, and project cumulative expenses. 2. Ensure that all products and/or communications with the Raising Hope logo must conform to the visual identification specifications and be pre-approved by the project manager. 3. Provide to the Commonwealth a final written report with a full accounting of expenditures relating to this grant within thirty (30) days after the last expenditure, and no later than thirty (30) days after the end of the term of this grant. Any dispersed funds that are unused shall be returned to the Commonwealth within this same time period. Any funds that will not be used by the Contractor must be reported to the Commonwealth sixty (60) days prior to the end of the term of this grant. Failure to timely submit the required report shall be considered a breach of this Agreement and may result in a demand for reimbursement of all grant funds to the Commonwealth.
