Kentucky Cancer Program East Outreach C4836 Scope of Work University of Kentucky Research Foundation (UKRF), Kentucky Cancer Program – East (KCP) shall: 1. Provide technical assistance/training to 3 health systems participating in the Centers for Disease Control and Prevention (CDC) funded “2002” grant to implement grant objectives related to the use of small media materials using resources from the American Cancer Society, Centers for Disease Control, or the National Colorectal Cancer Roundtable. Materials should be tailored to the needs of the patient population served by the health systems. 2. Attend monthly virtual and/or in-person meetings of the CDC 20-02 grant leadership team to report on progress and challenges in delivering support to the designated health systems. 3. Participate in monthly health system Technical Assistance (TA) support calls on a schedule to be mutually determined when the contract is in place. 4. Implement educational and awareness strategies targeting community organizations, health providers and interested partners about Kentucky Colon Cancer Screening Program (KCCSP). These strategies are aimed to improve knowledge of KCCSP among community partners with access to uninsured and underinsured Kentuckians eligible for KCCSP services. 5. Collaborate with the Kentucky Cancer Link (KCL) staff to coordinate complementary outreach efforts that inform Kentucky residents about free colon cancer screenings through the KCCSP, eligibility standards for the program and how to contact KCL staff to enroll. 6. Participate in monthly calls with the Department for Public Health (DPH) and KCL staff to review progress toward KCCSP deliverables – call schedule to be determined when contract is in place.
