Scope of Work
KCP at UK Markey Cancer Center has implemented lung health and lung cancer screening initiatives for
decades. Alongside our counterpart at University of Louisville, KCP East and West have collaborated and
engaged community stakeholders to address lung health and lung cancer screening burdens at the local
level. This leveraging of resources with partners allows KCP and its partners to operationalize and
customize strategies that best serve community-speci?c needs across Kentucky.
Building on successful models, KCP proposes to focus on evidence-based interventions and best practices
that improve provider and community stakeholder knowledge of lung cancer screening guidelines. The
focus audience may include hospitals, health systems, federally quali?ed health centers, primary care
providers, and other partners that provide services to the target population. Additionally, KCP will engage
district cancer councils, community partners, worksites and others to implement community and clinical
linkages that improve access to care for screening eligible Kentuckians, including collaboration with
Kentucky CancerLink for screening navigation services for uninsured and underinsured Kentuckians.
Deliverables/Services
KCP Regional Cancer Control Specialists (RCCs), 9% effort:
• Research and secure available state and national materials available for patient, public and
provider outreach programs. Develop new and update existing materials as needed.
• Implement education and awareness strategies targeting health care providers. These strategies
are aimed at improving lung health and lung cancer screening among eligible Kentuckians and may
include: provider/clinic training and education; provider and organization technical assistance, and
others.
• Conduct o?ce visits with primary care provider practices and federally quali?ed health centers to
share information and resources to help increase understanding of lung cancer screening eligibility
guidelines that facilitate screening referrals among patients. Create an encounter form to
document practice location, participating providers/sta?, and information/technical assistance
provided. Provide contact information for local lung cancer screening center(s) for referrals.
• Meet with district cancer councils and community partners to provide updates on lung cancer risk
reduction strategies, lung cancer screening including Kentucky statistics, patient eligibility
guidelines, insurance coverage, low dose CT screening, patient/provider shared decision making,
and other information.
• Engage community stakeholders and interested partners in developing and implementing a special
project in each Kentucky district to educate, recruit, refer and screen eligible patients as part of a
National Lung Cancer Awareness and Screening Campaign. Prepare a completed project report
including a list of participating partners, project strategies, number reached, and other relevant
project outcomes such as number referred for screening, and number screened.
• Collaborate with Kentucky Cancer Link to explore areas of need for uninsured and underinsured
patients seeking lung cancer screening.
• Provide updates at the Lung Cancer Screening Advisory Committee meetings as requested.
• Submit quarterly progress reports to the Kentucky Lung Cancer Screening Program.
