TIEH TEAM SOAR will utilize the SSI/SSDI Outreach, Access, and Recovery (SOAR) model to increase access to bene?ts, identi?cation, treatment, and recovery for the unsheltered homeless with serious mental illness (SMI), Serious Emotional Disabilities (SED), or Co-occurring Disorders (COD) in the Louisville Metro area. HDI will ensure that all data are collected, analyzed, and reported to SAMHSA as required. Additionally, the data and related analysis will be used to monitor the e?ectiveness of the e?orts, adjust implementation to increase e?ectiveness. HDI will provide regular updates to the grant’s project director and implementation team to guide quality improvement e?orts to ensure identi?ed behavioral health disparities are being addressed and progress is being made on closing the gaps.
