SCOPE OF WORK – HRSA GERIATRIC WORKFORCE ENHANCEMENT PROGRAM (GWEP) ACADEMIC PLACEMENT – SCHOLARSHIP SUPPORT SCOPE OF WORK: 1. Serve as the academic site to designate two (2) students to each receive a scholarship each semester (fall/spring) for practicum placement in the community/virtually, as arranged in coordination with the University of Louisville GWEP grant team; 2. Arrange for student account to be credited for financial aid provided for completing practicum placement with the Trager Institute as part of its Geriatric Workforce Enhancement Project. 3. Supervision of students will be conducted by faculty of the Trager Institute who are members of the GWEP grant team. 4. Documentation of completion can be made available to each respective institution of higher education upon request to comply with institution’s financial aid or practicum placement requirements. SCOPE OF WORK – HRSA GERIATRIC WORKFORCE ENHANCEMENT PROGRAM (GWEP) ACADEMIC PLACEMENT – SCHOOL OF SOCIAL WORK INVOLVEMENT SCOPE OF WORK: 1. Serve as the school leading practicum placements for social work students who will work in the community and virtually to implement FlourishCare Model of geriatric assessments and interdisciplinary approach to patient care; 2. Arrange for student scheduling and placement at community, primary care healthcare location, including virtual settings. Social work students will gain experience and practice through implementation of the Flourishcare Model he/she learns from the grant interdisciplinary team. The faculty will also facilitate arrangements necessary for student to receive scholarship payments. 3. Supervision of students will be conducted by faculty of the Trager Institute who are members of the GWEP grant team. The School of Social Work Field placement faculty will validate student practicum work with placement sites to ensure learning opportunities are available and offered to the student as part of practicum. Summary of involvement in practicum placement and validation of activities will be necessary to receive payment. Payment will be made in one lump sum fixed price payment in the final quarter of the academic school year.
