Of Pediatrio Room J402 Eentuoky Clinio 740 Limestone Lexington, EY 40536-0284 ATTN: Paula Cox (hereinafter referred to as "Contractor"). NOW THEREFORE, in consideration of the mutual agreements set forth herein, the two parties hereto, intending to be legally bound hereby, agree as follows: The Contractor assigns Tom Young, M.D. as the pediatrician and medical director for the Federally funded project at the school-based clinic at Harrison Elementary School to provide the following services: A. Pediatric outpatient primary care services and medical supervision of the nurse practitioner. Supervision of medical residents providing medical services in the School-based Center. Referral into the Lexington-Fayette County Health Department Primary Care Center System in accordance with appropriate protocols. Utilize all services available at the school-based clinic site and the Lexington-Fayette County Health Department Primary Care Center prior to referral to other outside services. Manage health records at the school-based clinic according to Health Department policy Assist with development and implementation of the school-based clinic services, including the health education and promotion program and evaluation plan according to the project proposal, at Harrison Elementary School. Provide data needed to meet grant reporting requirements, including financial data related to the project. Work collaboratively in writing future proposal for continuous funding of the school-based clinic at Harrison Elementary School. 1. B. C. D. 2. E. F. G. H.
