HCB Assessor 2015-16

1. Provide a team of Registered Nurse (RN) Case Managers and a team manager to complete HCB assessments. Provide qualified, appropriately trained individuals to perform the functions of the following positions: Registered Nurse Program Manager (1.0), Registered Nurse team (19), Project PI (.10). 2. Design and manage the project to support the delivery of the HCB Waiver program, in compliance with the plan approved by the Center for Medicare and Medicaid Services (CMS). • Design comprehensive assessment process including data collection forms and policy and procedures • Analyze and report on the needs and services for the waiver population • Evaluate the outcomes of the HCB project to validate the HCB waiver tools and collection information for the outcome measure established within the Waiver for compliance with the Centers for Medicare and Medicaid Services
