Health Center Outreach and Enrollment Supplemental Funding

UK North Fork Valley Community Health Center will align current resources (such as our Dental Outreach Program) with Outreach and Enrollment funding to target the 30% of children in the two county service area that aren’t covered by insurance. By working with Family Resource Centers in the schools and the Dental Outreach Program we will reach a larger audience who may require assistance in their home communities. We will support our current patients by contacting all those who utilized the sliding fee discount program in 2012 and 2013 to discuss coverage options and advertise the new service. We will update our web page to discuss new coverage options and provide a contact number for any individuals who may require assistance with enrollment. We will work with Homeplace Navigators (termed community health workers) to ensure that any patients they serve are aware of the service and expanded coverage options. We will also work with local community agencies that serve the uninsured (LKLP, KRCC, Health Department, Little Flowers Clinic, Community Ministries, etc) to ensure that those agencies are aware of the new coverage guidelines and the enrollment assistance that we will provide. Outreach workers will assist community members throughout all phases of the application and enrollment process.
