Home-Based Under-Mattress Monitoring Device to Estimate Mandibular Advancement in Oral Appliance Therapy for Obstructive Sleep Apnea

Abstract Titration of mandibular advancement device (MAD) represents a difficult task for clinicians given the fact that is typically performed according to patients’ subjective symptoms, which are known not to be a reliable method for determining treatment effectiveness. Over the years, different research has tried to overcome this, and even though promising results have been reported with the use of different overnight titratable devices, the results are mixed in terms of effectiveness and the technology is not widely available. There is a need for an easy-to-use, simple method that offers longitudinal measurements and can assist in the titration process of MAD. An under- mattress monitor (Fullpower Technologies) has already demonstrated high reliability in sleep measurements and sleep breathing events when compared to polysomnography. The aim of our study is to evaluate the effectiveness of the under-mattress monitoring device to aid in the titration process. A secondary aim is to evaluate inter-individual AHI variability over longitudinal nights of recordings, changes in subjective OSA symptoms and patient satisfaction. An additional aim is to analyzed differences in sleep parameters as recorded by the under-mattress monitor between responders and non-responders to therapy.
