Hope Center Health Care Clinic

The purpose of this award: To continue the operation and management of the Hope Center Health Care Clinic in accordance with established procedures and standards currently in place; To provide nursing staff and services at a level allowable by the grant budget; To collect and supply adequate data as determeind by the HRSA Grant, UDS Reports statistics and other informaton document services, meet reporting requirements, and evaluate the erffetiveness of the program; To assume responsiblity for appropriate referral in ot the Lex-Fayette Health Dept. Primary Care Center System in accordance with appropriate protocols; and To consult with the Primary Care Center Medical Director as needed.
