Description
Pediatric medical services provided by Tom Young, M.D. for Johnson Elementary School Clinic will include the following:
A. Serve as Medical Director for the Johnson Center.
B. Provide medical supervision for the Nurse practitioner at theJohnson Center.
C. Provide pediatric outpatient primary care services 4 hours a week, to include medical consultation and evaluation for children with learning disabilities and attention deficit disorders.
D. Supervision of medical residents providing medical services.
E. Participation at meetings of the professional multidisciplinary team, as needed for consultation on complex medical problems.
F. Participation with health educators on the health education curriculum.
G. Participation in development of a comprehensive school health program.
H. Participation in development and implementation of programs, based on needs assessment such as asthma or obesity programs.
I. Referral of students with no medical home to the Lexington-Fayette County Health Department primary Care Center for servicesnot available at the school clinics.
J. Dental referral of students to the Health Department's dental clinic who are patients of the Health Department.
