Description

The University of Kentucky (UK) shall: 1. Collaborate with local health departments and community partners at (6) sites funded by the KCCSP in the eastern part of the state to plan and implement education and outreach strategies to educate the target population about the KCCSP and facilitate recruitment for colon cancer screening. Activities will be tailored to specific communities based on funding and available resources. 2. Provide educational and outreach materials to promote the Kentucky Colon Cancer Screening Program (KCCSP) in the eastern part of the state at the (6) funded sites: a. to recruit appropriate clients for screening by identifying target groups, educating them about KCCSP, communicating eligibility, and making referrals to a medial home at one of the participating public health centers, b. that are customized for the community and structured to meet the needs of the specified service area, c. that announces the new KCCSP to the community, informs the target population about screening, and provides access information for the new service, d. that can be used for small group and one-on-one education about colon cancer, prevention, risk factors, colorectal cancer screening tests, the importance of screening, the new KCCSP program enrollment, and service locations, and e. that can be incorporated into National Colon Cancer Awareness Month activities. 3. Produce a panel card that can be distributed to potential clients that will: a. present information about colon cancer screening, the KCCSP, patient eligibility, and service contact information, b. be used for recruiting patients for screening and referral to a medical home, and c. provide a resource list of patient navigators including contact information at each of the public health centers to connect patients to a medical home and to the KCCSP. 4. Produce a panel card that can be distributed to partners/stakeholders, especially those who currently work with the target population: a. to share key messages that describe the KCCSP, b. to extend reach and facilitate linkages, and c. to facilitate provider education in KCCSP communities. 5. Work with health department staff to identify treatment resources and reduce barriers using the KCP Pathfinder. In year two of the contract, the KCP will collaborate with local health departments and community partners at the (6) sites funded by the KCCSP in the eastern part of the state to expand education and outreach strategies based on funding and available resources. Activities will be tailored to specific communities and could include the following: 1. Encourage the incorporation and integration of education about the KCCP into existing activities to facilitate awareness and recruitment of the target population. 2. Provide a press release for the local newspaper. 3. Provide public service announcements for radio and TV. 4. Secure billboard promotion and advertising. 5. Utilize social media to increase awareness about KCCSP education and outreach activities.
