Data matching of physicians within Kentucky outside of the Greater Louisville (as defined by KHC) to the CMS data and the provider files for proper group attribution. Data verification through correspondence with provider website or phone call to site. Deliverables to be included are csv files for the following: 1. Group file- state group practices as provided in the template labeled Groups_2014. 2. Physician file- providers mapped to group practice as provided in the template labeled PG2014.
