Kentucky Homeplace Project - FY'18

1. Conduct at least (30) CDSMP courses between July 1, 2016 and June 20, 2017 and submit data on participants with reimbursement based on $25 per participant. In FY18 conduct at least (30) CDSMP courses between July 1, 2017 and June 30, 2018 with reimbursement based on $25 per participant. 2. Develop and maintain an electronic list of trained CDSMP leaders and course schedule and update list as needed. This shall be in a DPH approved format. 3. Develop and maintain an electronic list of trained WWE course leaders and course schedule and update list as needed. 4. Provide a list of trained AFEP, when offered, course leaders and course schedule and update list as needed. 5. Report aggregate data on CDSMP, WWE and/or AFEP program participants to the Kentucky Department for Public Health by January 30, 2017 and again by June 30, 2017 using CDC approved format to be provided. For FY 18, Report aggregate data on CDSMP, WWE and/or AFEP program participants to the Kentucky Department for Public Health by January 30, 2018 (covering July 1, 2017 – December 31, 2017); and by June 30, 2018 (covering January 1, 2018 – June 30, 2018), using CDC approved format to be provided. 6. Provide written documentation on how outreach/recruitment of program participants is conducted. 7. Provide written documentation on how outreach to health care providers for program referral is conducted, including list of providers/practices contacted, outcome of contact and referral process or materials developed. This shall be due by November 30 and May 30. 8. Purchase materials needed by CDSMP leaders including: (a) instructor materials: CDSMP Leader’s Manual; one per leader; (b) course required CDSMP participant books “Living a Healthy Life with Chronic Disease”; (c) course required relaxation CDs for participants. 9. Purchase WWE books as needed. 10. Participate in conference calls with arthritis program staff as needed. 11. By December 31, 2017, develop online programlocator that includes, but not limited to, arthritis appropriate evidence-based interventions.
