ABSTRACT
The System of Care Expansion and Sustainability Grant will build upon Kentucky’s long history
of developing a comprehensive system of care (SOC) for children and youth (birth through age
21), who meet criteria for having a serious emotional disability (SED), and their families by
expanding infrastructure and service delivery to those with child welfare involvement. The
evaluation of the program will include the formative evaluation of the objectives and summative
evaluation will include the impact of the program on improving the interagency infrastructure,
increasing availability and access to care.
