The University of Kentucky Prevention Research Center will assist the Kentucky Department of Public Health, Kentucky Tobacco Prevention and Cessation Program, with the development and implementation of an evaluation plan for two Affordable Care Act Supplemental Grants for Tobacco Control. The UK PRC will review action plans and products, meet with Tobacco Program Staff to review recommendations, and create an evaluation plan that captures outcomes and measures for each supplemental grant. The UK PRC will: 1) Participate in in-person meetings or conference calls to review timeline and grant documents; 2) Submit a written draft report that describes evaluation of the Kentucky Tobacco Prevention and Cessation Program, including a description of program activities, data sources, logic model, baseline data, current data, and gaps in data and recommendations for future data collection efforts for program evaluation; 3) Submit a draft of the Final Performance Report that includes progress towards objectives, success, barriers encountered, technical assistance and training requests; 4) Meet with Tobacco Program staff to review required documents, including the 2 Funding Opportunity Announcements, KY grant application including the action plan, and any publications produced with grant funds; 5) Deliver final documents, the evaluation plan and a report on the accomplishments of the Tobacco Program, to Tobacco Program Staff, for submission to CDC.
