Description
Provide an implementation and evaluation team to administer the MFP program for the KY Cabinet for
Health and Family Services. The project staffing plan consist of 3 regional teams composed of a social
worker program coordinator and a nurse program coordinator to implement the MFP program at sites
across the state. Implementation of the MFP program consist of contacting and interviewing potential
clients, assisting clients with an MFP application, assisting clients with home modification and household
startup processes, and monitoring and evaluation once placed in a community setting.
