Description

The Bluegrass Care Clinic (BCC) at the University of Kentucky seeks to facilitate the provision of quality care and services to people living with HIV and AIDS (PLWHA) by continuing to provide medical case management and care coordination services for patients receiving medical care at the Bluegrass Care Clinic and residing in one of the 31 counties identified in this solicitation as the Cumberland Valley Region for the Kentucky HIV/AIDS Care Coordination Program. The Ryan White Part B (RWB) Care Coordination program at the BCC currently facilitates medical and social support services for HIV positive patients living in central and eastern Kentucky and offers a holistic, multidisciplinary approach to promote self-sufficiency and medical treatment success. HIV Care Coordinators (HIV CCs) at the BCC serve as medical case managers that facilitate patient care by ensuring patients are able to participate in HIV treatment without barriers and that the social determinants of health are examined and addressed as important factors in medical treatment. The HIV CCs assist patients in applying for any eligible benefits (i.e. Medicare, Medicaid, Social Security, and ACA Exchange Based Insurance) and coordinates enrollment in Kentucky AIDS Drug Assistance Program (KADAP) and the Kentucky Health Insurance Continuation Program (KHICP) to facilitate health insurance premium assistance. Additionally, the HIV CCs serve as gatekeepers for a variety of core medical and support services, such as: outpatient/ambulatory medical care, emergency medications (non-KADAP formulary medications or pending insurance/patient assistance program approval); mental health counseling and substance abuse services; transportation assistance to medical appointment (gas cards, WHEELs/cab vouchers & bus tokens –in amounts relative to time/distance travelled); medical nutrition therapy (i.e. Ensure, Scandishake); and other medical necessities such as durable medical equipment, glasses and medical supplies, etc. This application proposes that the BCC continue to provide care coordination and medical case management services to PLWHA in the Cumberland Valley region.
