Psychiatry Outpatient Clinic (C2279): CL 2: KORE-SOR Outpatient SORII (TABJ)

FY22 KORE-SOR Outpatient Deliverables 1. Sustain the outpatient clinic at 245 Fountain Court in current UK Psychiatry occupied space. 2. Create a dedicated clinical space at 245 Fountain Court to include four to five (4-5) treatment rooms, a group therapy room, laboratory, restroom, waiting area, and separate entrance for this clinic. Fund waiting room and treatment room furniture per UK HealthCare Ambulatory standards. 3. Establish data management infrastructure for tracking of required metrics and outcomes to include partial funding for a person dedicated to data entry and tracking along with required computer hardware and software. Purchase of a tablet device for capturing data from patients while in the clinic. 4. Establish an incentive mechanism for research participation for follow-up interviews. 5. Establish linkage and prioritize admission of persons with OUD discharged from Chandler Hospital as well as UK HealthCare Good Samaritan Hospital. 6. Hire two (2) Certified Alcohol and Drug Counselors (CADC) who shall provide ongoing substance abuse treatment counseling and management for clinic patients. 7. Hire one (1) nurse practitioner who shall provide buprenorphine treatment to patients as well as assist the clinic leadership with administrative compliance with all applicable state and federal regulations. 8. Hire clinical staff to schedule and register patients, conduct urine drug screening in accordance to established protocols and orders created by the physicians and/or nurse practitioner and assist the providers during group therapy sessions. 9. Identify the following providers who shall deliver clinical services in the outpatient clinic: a. Two (2) part-time physicians. b. Two (2) LCADCs/Licensed Clinical Social Workers (LCSW). c. One (1) Addiction Fellow. 10. Conduct clinical assessments on an outpatient basis by licensed professionals, including buprenorphine waivered physicians and Nurse Practitioners and other staff as appropriate. Assessments will follow the Substance Abuse and Mental Health Services Center for Substance Abuse Treatment guidelines for Medication Assisted Treatment (MAT) and include a full Diagnostic and Statistical Manual (DSM) assessment as well as medical and psychiatric history. 11. Provide transportation assistance to and from required appointments, patient education materials, as well as supplies for basic needs such as toiletries and other essential items. 12. Provide Naloxone (Narcan) Kit and/or prescription plus overdose prevention education to the patient, family member, or other individual designated by the patient. 13. Ensure that all care provided by the outpatient clinic shall be evidence-based or consistent with good clinical practice and compliant with Kentucky Board of Medical Licensure (KBML) and federal regulations. 14. Ensure linkage to other services for patients who require a higher level of care than outpatient treatment. 15. Maintain hours of operation during regular business hours, Monday through Friday 8 a.m. to 5 p.m., at the launch of the clinic. Hours of operation may shift in response to patient need. 16. Incorporate peer support specialists into the treatment plan as appropriate per physician or Advanced Practice Registered Nurse (APRN) orders. 17. Acknowledge federal funding in all communications activities using the following tag line: “This project is supported by the Kentucky Opioid Response Effort (KORE) through a Substance Abuse and Mental Health Services Administration (SAMHSA) Grant 1H79TI081704 H79TI083”. 18. Hire one (1) pharmacist who shall be an educational resource and provide counseling to clinicians and patients as it pertains to substance treatment and common co-occurring medical (i.e., HCV) and psychiatric co-morbidity. The pharmacist will also assist as a liaison between the clinic and pharmacy services and will assist with compliance with all applicable state and federal regulations. 19. Provide injectable, long-acting buprenorphine (e.g., Sublocade) for appropriate patients until prior authorizations are received and in place.
