Abstract
UKRF, KY REC shall:
1. Support two health systems to increase referrals to Quit Now Kentucky
2. Assess current levels of connectivity and program usage.
3. Develop an action plan to improve workflow, usage, and referrals to Quit Now Kentucky.
4. Lead interventions designed to integrate Quit Now Kentucky into the practices'' workflows, thereby boosting
overall program usage.
5. Lead Plan, Do Study, Act cycles to identify barriers and successes.
6. Provide education and workflow development to facilitate increased usage and referrals.
