The UnIversity will provide consultation, trainmg and support for the following projects, as detailed in this agreement BELONGING IN THE COMMUNITY To provide coordination and administrative oversight of the Supported living Program, and wiJJ assist the Division of Mental Retardation in establishing baseline measures and ongoing evaluation of the service system (Core Indicators Project). REAL CHOICES The UK currently funded project, entitled "Core Indicators Project (ClP)" will be enhanced through this project to recruit and train self·advocates and family members to be members of two-person interview teams to solicit feedback on consumer satisfaction with the Quality of services and supports for individuals with disabilities SUPPORTS FOR COMMUNITY LIVING To provIde the staff complement to the Division of Mental Retardation :0 assist in the administration of the Medicaid Waiver program for individuals with mental retardation and other developmental disabilities ADULT PKU CLINIC To locate patienls lost to PKU follow-up and provide information and referral mformation and assislance to Individuals as needed. Individuals hired under this contract shan be reimbursed for travel expenses in the same manner as classified state employees in accordance with 200 KAR 2:006, and shall not be considered to be in the category of individuals listed in 200 KAR 2:006 Section 8. Day'menl Terms: Monthly, based upon invoices submitted in accordance with allached budgets. TOTAL AMOUNT OF AGREEMENT IS 52.335.48400
