Scope of Work
Obtain Local IRB Approval/ Exemption for the project entitled: Real world experience on the use of
ceftolozane-tazobactam in the treatment of Gram-negative bacteremia, including septic shock - a
multicentric experience from the United States
After local IRB approval, the site will review all patients that have received the study drug “ ceftolozanetazobactam”
during time period of 1/1/2016 to 2/2/2023 and conduct a full chart review to determine
patients that meet the inclusion criteria. Site will review all eligible patient’s data for inclusion in the
University of Florida’s study dedicated RedCap database. The site will be responsible for patient
enrollment.
After data collection at the Site is complete, Site PI will review all collected data for completeness and
accuracy before submitting data to UF. All protocol required data variables will be recorded in the
University of Florida’s RedCap database, which will be designed and provided by the UF Principal
Investigator. Further edits/clarifications to any submitted data variables will be considered by the Site as
determined by the University of Florida Principal Investigator. Site will re-review the provided data
elements or any clarifications needed.
Site is authorized to enroll and provide data on up to 40 patients.
