Description
The purpose of the Money Follows the Person (MFP) program is to identify, contact, and assist clients in
nursing facilities or intermediate care facilities for individuals with mental retardation (ICF/MR) facilities to
relocate back to a community setting. Under the agreement, the University of Kentucky Institute for Biomedical
Informatics (UKIBI) shall provide CHFS with professional staffing to conduct research and support the delivery
of the MFP program, monitor and evaluate clients who have moved to community setting, develop and
maintain information technology systems for tracking client referrals, application and assessment to determine
client relocation, evaluation of home modification and household start-up needs, audit of program
expenditures, and program evaluation and CMS reporting.
