Description
The purpose of the Money Follows the Person (MFP) program is to identify, contact, and assist
clients in nursing facilities or intermediate care facilities for individuals with mental retardation
(ICF/MR) facilities to relocate back to a community setting. Outcomes which will be measured
during the conduct of the research include transition process, length of time required to
transition, eligibility characteristics, quality management, quality of life survey assessment; home
modification requirements; and overall cost and services rendered. The specific aims of the
research outcomes for the MFP Program is to 1) design, 2) implement, and 3) evaluation the
effectiveness of the project. The hypothesis which will be tested is that the recipients who
successfully complete the MFP transition will result in lower health care expenses and attain
improved physical and behavioral health outcomes. Under the agreement, the University of
Kentucky Institute for Pharmaceutical Outcomes and Policy (UKIPOP) shall provide CHFS with
professional staffing to conduct research and support the delivery of the MFP program, monitor
and evaluate clients who have moved to community setting, develop and maintain information
technology systems for tracking client referrals, application and assessment to determine client
relocation, evaluation of home modification and household start-up needs, audit of program
expenditures, and program evaluation and CMS reporting.
UKIPOP shall conduct research in the following areas: 1) Audit the conformance of the MFP
plan compared to the plan approved by the Centers for Medicare and Medicaid Services (CMS),
2) Conduct quality assurance audits on the assessments for relocation transition
determinations, 3) Conduct a complete program evaluation on the effectiveness of the program
4) Conduct patient-reported quality of life (QOL) and satisfaction surveys with participation in
MFP, 5) Analyze and disseminate patient reported outcomes on the QOL surveys, 6) Analyze
and compare Kentucky and other state transition programs.
