Description

Specific aism of project include improving the quality of life and CVD risk factor outcomes in rural Appalachian Kentucky using an individualized heart health intervention designed to promote self-management of CVD risk factors. This project addresses both of the two major goals of Healthy People 2010 by targeting cardiovascular disease (CVD) and prevent its progression in those who already have it. In doing so, the project will help to increase the quality and years of a healthy life of Kentuckians (Health People 2010 goal #1). The project will target people living ina rural, underserved area of Appalachian Kentucky (ie Breathitt and Perry counties) in order to target health disparities related to CVD and CVD risk ractors in this population (Healthy People 2010 goal #2).
