This cooperative agreement program provides: Pathways to Housing’s Housing First model; Assertive Community Treatment; Screening, Brief Intervention, and Referral to Treatment (SBIRT) and SSI/SSDI Outreach, Access and Recovery (SOAR) to attain the goal of eliminating homelessness. Four community based teams, made up of a multi-disciplinary outreach crew seek out the homeless wherever they might be. The 470 of the most seriously in need persons are targeted by CABHI Kentucky. The Evaluation Unit will provide performance, process and outcome data. The evaluation results will be provided to SAMHSA, the KDBHDID Steering Committee and KICH. Another Key component of this project is The Kentucky Opioid Response Effort (KORE) initiative. This initiative to provide a comprehensive targeted response to Kentucky’s opioid crisis by expanding access to a full continuum of high quality, evidence-based opioid prevention, treatment, recovery and harm reduction services and supports in high-risk geographic regions of the state. Informed by data on populations most in need, the KORE projects will focus on four primary populations: 1) opioid overdose victims, 2) pregnant and parenting women, 3) individuals re-entering society upon release from criminal justice settings and 4) adolescents and young adults.
