Kentucky Homeplace Abstract Purpose and Background This contract serves two purposes. First, to maintain operation of the Kentucky Homeplace Project to educate and assist rural Kentuckians, who live in medically underserved areas, in accessing appropriate health care, including preventative care. The project shall focus on the use of Community Health Workers (CHW) to provide these services. CHWs complete needs assessments of individuals and families in order to address access of immediate medical and/or social issues in the most cost effective manner. Kentucky Homeplace will address the following goals: 1. To educate Kentuckians to identify risk factors and use preventative measures to become a healthier people with knowledge and skills to access the healthcare and social system. 2. To find solutions to bridge the gap between uninsured and underserved individuals and the health care delivery system. 3. To educate and assist rural Kentuckians who live in medically underserved areas with appropriate health and preventative care. To utilize, CHW, in education and assisting rural Kentuckians with appropriate health and preventative care. SCOPE OF SERVICES The University of Kentucky Research Foundation (UKRF), Center of Rural Health Kentucky Homeplace shall: 1. 1. Provide education and assistance to rural Kentuckians. 2. Provide DPH a line-item budget for the upcoming Fiscal Year, by May 30th each year. 3. Provide DPH a detailed definition of each service, including chronic disease and senior assistance provided by the organization, within ten (10) days of contract implementation. 4. Provide a Plan of Action for each client; a. Each client will receive an initial assessment (as applicable) to develop an action plan for the clients served. Each plan of action shall include a summary of services provided and outcomes as indicated to be included in the quarterly reports. 5. Collect the following data within thirty (30) days of contract implementation: a. Type of payer (insured, Medicaid, Medicare, dual-eligible, uninsured) information. b. Information on hospital admissions and Emergency Department (E.D.) use for each patient. c. Identification of the type of visit (home visit, office visit, phone contact). d. The length of time for each visit actually spent with the patient (exclusive of any travel). e. The length of time spent by staff for training, community meetings, administrative processing (mailings, paperwork, etc.) separate from patient service time. 6.
