The University of Kentucky College of Social Work (UK CoSW) will subcontract with Collaborative Safety, LLC to provide technical assistance (Phase 2) for the Kentucky Department for Community Based Services¡¦ (DCBS) Systemic Critical Incident Review System. Technical assistance from Collaborative Safety, LLC will include: Conference/Virtual Meetings (up to 3 hours/Month) o Individual / Team Coaching o Progress Updates o Needs Assessment Unlimited email access Critical Incident Report ¡V Quality Review o Skill Support Technical Data Review Learning Points Development Human Factors Debriefing Systemic Mapping Narrative Creation Scoring Unlimited access to Collaborative Safety¡¦s Systems Mapping Software Quarterly Onsite Visit (Two Days/Quarter) o Support as Needed (TA/Training/Mapping Support)
